ANNUAL SAFETY AND HEALTH REPORT AT WORK

Semiannual report on the activities undertaken during the period 2014 - 2015, concerning the implementation of the Management System Occupational Safety and Health, according to the provisions of the Law on Safety and Health at Work - Law 29783 (hereinafter LSST ) and its Regulations - DS. No. 005-2012-TR (hereinafter RLSST) and the baseline report as follows.

According to the provisions of Article 40 ° of the LSST, it is necessary that the Management System Safety and Health at Work is subject to periodic assessments to measure their results in this way to achieve continuous improvement. This report monitors progress regarding the implementation of the Management System Safety and Health at Work in the company in this last semester.
III SOURCES OF INFORMATION
In preparing this report we have used the following information:
1. Physical and electronic records of activities during the semester Implementation of Safety Management System and Health at Work.
2. Documentation Management System Safety and Health at Work.
With the information that has been obtained from both physical records and electronic records, it was possible to determine the following:
1. ELECTION SUPERVISOR SAFETY AND HEALTH AT WORK
One of the main activities developed by the working group during the first six months of implementation was aimed to establish an electoral process in order to choose a Safety and Health at Work Supervisor in accordance with the guidelines established by the LSST and RLSST. Through this electoral process, Safety and Health at Work supervisors (and alternate) were chosen for the period August 2014 - August 2016 for the following companies HPO group.

2. POLICY HEALTH AND SAFETY AT WORK
During the first six months of the Safety and Health at Work Management System implementation a Security and Health at Work Policy was developed, taking into account the following guidelines established by Law:
a) To be specific to the organization and appropriate to its size and nature of its activities.
b) To be concise, clearly written, dated and made effective by the signature or endorsement of the employer or senior representative with the highest responsibility in the organization.
c) To be communicated and readily accessible to all people in the workplace.
d) To be regularly updated and made available to external stakeholders, as appropriate.
Legal basis: Article 22 of Law 29783; paragraph a) of Article 32 of the DS. 005-2012-TR - REGULATION.
3. IPER - HAZARD IDENTIFICATION AND RISK ASSESSMENT
To date, the Safety Engineer has gathered information from work activities carried out at the premises of the company, which enabled Him to determine the risks associated with them, so that on that basis the necessary strategies for control or elimination of risk can be taken.
Legal basis: subsection c) of Article 32 of the DS. 005-2012-TR - REGULATION.
4. PLANNING PREVENTIVE ACTIVITY
During the year of implementation of the management system, various activities destined to disease prevention purposes and elimination of risks in the workplace were developed; these tasks have been classified as follows:

5. PROGRAM ANNUAL SAFETY AND HEALTH AT WORK
An annual Program for Safety and Health at Work was developed, proposed to the Management and Work Supervisor and further adopted; in order to organize and plan monthly activities during the year. Legal basis: paragraph f) of Article 32 of the DS. 005-2012-TR.
6. RECORDS MANAGEMENT SYSTEM MANDATORY SAFETY AND HEALTH AT WORK
Mandatory records for Safety and Health at Work Management System were implemented. Those documents record all activities during the year referred to the Management System Safety and Health at Work, which include training incidents, accidents and, provision of personal protective equipment; among others.

The training was performed by Occupational Physician Jim Llenque and addressed to company employees, during this activity a practical recognition of first aid kits and related tools took place according to technical specifications established by the G050.
A practice known as cardiopulmonary resuscitation named CPR was also performed, it should be noted that the proper use of this technique can help maintain the life of an injured person until the arrival of paramedics.

Company workers participated actively in practicing resuscitation techniques taught by the occupational physician, simulating accidents in order to evaluate their performance in emergency cases, good results related to workers performance were achieved.

8. PROCEDURES AND PROTOCOLS
06 procedures and protocols have been developed and implemented, and have been made available to the company for its full implementation, it should be noted that these procedures and protocols once released will guide all personnel specific acts to any type of situation.
The procedures that have been incorporated into Management System Safety and Health at Work, are:
• Occupational medical examinations
• Respiratory protection
• Response to medical emergencies in the work area
• Investigation of occupational diseases
• Procedure for risk management for pregnants
• Manual for use kits
